Getting Started:
Prior to move-in, a number of forms must be completed.  Please contact our admissions coordinator to make arrangements to complete these forms

What to Bring
The following information is necessary to complete the move-in forms:
  • Full Name
  • Social Security Card
  • Insurance Card
  • Birth Certificate or driver’s license
  • Birth place
  • Attending physician’s report including physician’s name, address, and telephone number
  • Pharmacy name, address and telephone number
  • Advance directives (living will or durable power of attorney for health care

Clothing
Appropriate Clothing is encouraged, including pants and shirts for men, slacks, blouses, dresses and skirts for women.  Athletic suits are ideal for both.  A minimum of five changes of clothing should be provided and all clothing must be clearly identified with labels. Items should be selected for ease of maintenance, comfort and durability and should be easy to put on and take off. 

Furniture and Personal Belongings
Furniture and personal items brought in for residents use will be labeled and noted on the resident’s inventory list contained in the medical record.  Likewise, all items removed must also be noted on the resident’s inventory list. The rooms are fully furnished with a bed, dresser, mirror, nightstand, lamps and small refrigerators, there is no need to bring any furnishings.  However, residents may bring a small T.V set and may furnish their private rooms as they desire, with the approval of the House Manager. 

Although the grounds are covered by general liability insurance, this insurance does not cover any property belonging to a resident.  Residents are, therefore encouraged to carry personal property insurance to cover any loss due to fire, theft, or other casualty.  It is not recommended that residents bring valuables such as jewelry to the facility (costume jewelry is OK) and we ask they keep only a maximum of $50 cash in their possession. Each resident room is equipped with an in-wall safe with lock and key and residents are encouraged to lock all valuable items.  However, Highland Circle still cannot be responsible for any loss of money, jewelry or other valuables. 

Pets
A resident may be allowed to have a personal pet with advance approval from the House Manager.  A pet may initially be accepted into the Home on a “trial basis” only after a pet assessment has been performed.  Admission of a pet depends on such factors as size, noise factor, odor, behavior, and cleanliness.  Proof of a healthy animal and required vaccination from a veterinarian will be required before the pet comes to stay at Highland Circle

Highland Circle reserves the right to ask that the resident or responsible party find outside living arrangements for the pet, if it is determined that the pet is destructive, disruptive, or poses a health or safety hazard to the Home or its other residents.  The resident must be solely capable of properly caring for the pet,  including feeding, grooming, and clean-up.  An additional amount may be charged as a non-refundable Pet Deposit and/or pet rent.
